How to send a case to Precision Dental Restoration

Download the PDR Rx and clearly fill it out with your preferences.

Case materials must be in sealed bags.
Models should be wrapped carefully with bubble wrap or foam to avoid damage during shipping.
Include photos in a separate plastic bag.
When shipping an articulator and facebow, remove the models from the articulator and wrap individually.
When shipping alginate, seal Rx forms in seperate plastic bag - Attach the Rx bag to the case bag.
When shipping loose crowns, bridges, implant parts and/or shade tabs, place small parts in a separate container or sealed plastic bag and staple to Rx. Include the patient’s name on the additional sealed bags attached to the Rx.
When shipping more than one case in a box, please bag each case separately.

Patient’s Last Name and Initial are on the Impression/Models.
Complete Prescription form for Every Case. Blank Rx forms can be mailed to your office (FREE of charge) or can be downloaded through our website.
Securely seal box with tape. If recycling or reusing a box, be sure to cover old mailing labels to ensure safe delivery.
